Indication:
REVUMENIB, of chorea associated with Huntington’s disease and for the treatment of relapsed or refractory acute leukemia in adult and pediatric patients aged 1 year and older who have a documented rearrangement of the KMT2A gene (also called KMT2A-translocation). More recently, the indication was expanded to include patients with relapsed or refractory acute myeloid leukemia (AML) harboring a susceptible NPM1 mutation who have no satisfactory alternative treatment options. This targeted therapy is specifically for those whose leukemia has this genetic marker (KMT2A rearrangement or NPM1 mutation) and who have already tried other treatments that failed or stopped being effective.
Dosage:
The recommended dosage for Revumenib (brand name Revuforj) for adults and children aged 1 year or older with relapsed or refractory acute leukemia harboring a KMT2A rearrangement (and in some cases an NPM1 mutation) is twice daily (every 12 hours) until disease progression or unacceptable toxicity. For patients weighing 40 kg or more, the dose is 270 mg orally twice a day (without strong CYP3A4 inhibitors) and reduced to 160 mg twice a day if a strong CYP3A4 inhibitor is concurrently used. For patients weighing less than 40 kg, the dose is body surface area (BSA)-based: without strong CYP3A4 inhibitors it is 160 mg/m² twice daily, and with strong CYP3A4 inhibitors it is reduced to 95 mg/m² twice daily; for example a BSA of 1 m² corresponds to ~160 mg twice daily in the first scenario.
Extra note:
Prior to starting Revumenib, the white blood cell count should be reduced to < 25 × 10⁹/L and an ECG should be performed because of the risk of QT-interval prolongation, and patients must be selected based on documented KMT2A rearrangement (or in the NPM1 case, a susceptible mutation). The drug may be held or dose-reduced for severe adverse effects (e.g., severe neutropenia or QT-prolongation) and should be continued for at least 6 months in responding patients (without progression/toxicity) to allow time for clinical benefit.
Note:
Revumenib is contraindicated in patients with untreated or active depression, suicidal ideation, hepatic impairment, or those taking monoamine oxidase inhibitors or reserpine. Regular monitoring of mood, motor symptoms, and cardiac status is recommended during therapy.
